POST RENAL TRANSPLANTATION

-Normal size and echotexture of transplant kidney

-No evidence of renal artery stenosis

DERMATOLOGY REFERAL-

C/o itchy skin lesions over both buttocks,back,abdomen,thighs since 5 months(on and off)

H/o topical medication (LULICONAZOLE)

Diagnosis- TINEA CORPORIS ET CRURIS

RX- (1) TAB.TERBINAFINE 250 MG O/D X 1 WEEK.      ------0------

(2) ZODERM-E CREAM L/A   O/D.   X 2 WEEK

       Morning and night

(3) TAB.TECZINE 5 MG.   OD X 2 WEEKS

       Night.

(4) LULIFIN CREAM L/A. O/D.  X 2 WEEKS

       night

DENTAL REFERAL-

Ptient is referred to dept of cons for restoration in relation to 1/7.     2/7.    3/8.    4/8.   To dept of OMFS for extraction in relation to  2/5.   To DEPT OF PERIODONTIS FOR ORAL PROPHYLAXIS.

OPHTALMOLOGY REFERRAL-

-Normal fundus study
