Patien came to casuality with 

C/o pain abdomen since 2 years.

Pain in legs since 4 years.

HOPI- 

Patient was apparently normal 4 years back when he developed pian in legs.

Later developed pain abdomen(epigastrium) radiating to chest 

A/w vomitings and burning sensation.

K/c/o kidney stones (on medication-homeopathy)

No h/o DM,HTN,ASTHMA,EPILEPSY.

DIAGNOSIS: 

 ACUTE FISSURE-IN-ANO

TINEA CORPORIS OF CRURIS +POST INFLAMATORY LEUCODERMA.

DERMATOLOGY REFERAL:

Itchy skin lesion over the buttock since 4 years,. C/o pigmentation under eye since 1 year, H/o white coloured skin lesion over lower lip since 1 year.

TINEA CORPORIS OF CRURIS +POST diagnosis- INFLAMATORY LEUCODERMA.

(1) FLUTIVATE CREAM L/A  B/D X 2 WEEKS

       -morning and night. (lips)*

(2) LULIFIN CREAM  L/A B/D.   X 2 weeks

       Morning and night (Buttocks/Tinea)

(3)  Tab.TECZINE 10 MG /OD X 2 WEEKS.

SURGERY REFERAL-

Complaints  of pain in anal region while passing stools, no H/o bleeding P/R , h/o on and off constipation (hard stools)
