Amc case, new admission
Patient was brought to casuality with c/o, fever since 3 days, and sob since today evening.
HOPI -
patient was apparently asymptomatic 3 days back then he developed fever, high grade a/w chills and rigors, intermittent, more at night
Later on the patient developed SOB, recently since the evening of 28/12/21 at around 8 - 9
No H/O Haemoptysis,chestpain,cold,
No H/o. Hematuria,rashes over the body, melena etc
No h/o weightloss,nightsweats.
Past history-
Patient is not a k/c/o DM, HTN, CVA, CAD, EPILEPSY, BA
GENERAL EXAMINATION :-
Pt is c/c/c;
PICKLE -
Vitals on admission
PR - 118 bpm
BP - 100/70 mm Hg
RR - 22 cpm
Temp - 103 F
Spo2 - 95 % RA
SYSTEMIC EXAMINATION :-
CVS - S1, S2 +
RS - BAE +, crepitations +, left IAA,
P/A - soft non tender
CNS - no FND, GCS (15/15)
DIAGNOSIS: VIRAL PYREXIA WITH THROMBOCYTOPENIA WITH VIRAL PNEUMONIA. (RESOLVED)
Advice at discharge:
(1) TAB.AUGMENTIN 625 MG/PO/BD

(2) TAB.AZITHROMYCIN 500 MG /PO/OD
